SMART Rehab, a signature rehabilitative care program developed by Ocean Healthcare, has set a new standard of superior care and comfort unparalleled in the healthcare industry.

Heart SMART

South Mountain’s HeartSMART program is the first of its kind in a post-acute setting, providing a seamless continuum of cardiac-focused care upon discharge from the hospital.

The Heart SMART program accommodates patients with a broad range of cardiac conditions, including congestive heart failure, post-myocardial infarction, post-coronary artery bypass and valve replacement, and post-cardiac catheter intervention.

Admissions

South Mountain accepts admissions seven days a week. We accept Medicare, Medicaid, and many insurance plans, as well as private pay. Our Director of Admissions can assist you with insurance and Medicare/Medicaid guidelines, and help you in maximizing your benefits.

Our staff works closely with hospitals, doctors, discharge planners and social workers to ensure a smooth and easy transfer to Hampton Ridge Healthcare and Rehabilitation.
